Transition to Independence
"A Home of My Own" Supports Brokers and Life Coaches support people with intellectual disabilities, autism and other disabilities in their own homes and in their family's homes, the latter which is called "Transition to Independence". The companion and in-home community living services can be used in both a family home and a person's own home.
Transition to Independence is a wonderful opportunity for:
- People who, emotionally, are not quite ready to move out of the fammily home
- Families who, emotionally, are not quite ready for their family member to move
- People who need to learn additional community living skills to safely live in their own home
